Create an RCS card from the builder (Tier 3

Builds a Twilio Content Template (twilio/card + twilio/text fallback) from a builder payload and registers it in rcs_content_templates so it appears in the composer picker. Uses the workspace's own Twilio credentials. Enforces RCS card limits (title ≤200, subtitle ≤60, body ≤1600, ≤4 buttons). Authenticated workspace user (Bearer).
