Approve a proposed training example into the live prompt

Owner/admin/manager only. Proposals (is_approved = false) are generated from QA-passed Field Coach conversations and are ignored by every AI runtime reader until approved here. Approving an already-approved example is a 404, so a double click cannot rewrite its attribution.
